Formm Terms of Service

1. About these terms

These terms are an agreement between you and Avlo Ltd, registered in England and Wales (company number 17095752), 34 West Woodside, Bexley, DA5 3PQ, United Kingdom ("Formm", "we", "us"). By creating an account or using the Formm app you agree to them. If you do not agree, do not use Formm. Our Privacy Policy explains how we use your personal data.

2. What Formm is

Formm is a strength training app. It builds training plans and writes notes from an automated coach using the information you give it, including information about your injuries, your training and your food. The coach is an automated system, not a person.

3. Not medical advice

Formm is coaching, not medical advice. Nothing in the app, including plans, form tips, injury-related adjustments, check-in responses and food estimates, is a diagnosis, treatment or medical, physiotherapy or dietary advice, and it does not replace a doctor, physiotherapist or registered dietitian.

Speak to a suitably qualified professional before you start or change an exercise or nutrition programme, especially if you are injured, pregnant, unwell, taking medication or have a medical condition.
You are responsible for deciding whether an exercise is suitable for you. Stop if something hurts or feels wrong, and get advice.
Plans and notes are generated automatically and can be wrong. Calorie and macro figures are rough estimates.

4. Your account

You must be at least 18 years old, and you confirm that when you create your account.
Give accurate information and keep your password secure. You are responsible for activity on your account.
Tell us at once if you think someone else has accessed it.
You can delete your account at any time from Profile > Delete my account.

You choose what to tell Formm. Some of it is health information. We ask for your explicit consent to use it to personalise your coaching, as described in the Privacy Policy, and you can withdraw consent at any time by deleting information, turning off the AI coach in Profile, or deleting your account. Without it we may not be able to personalise coaching.

Health Connect and Apple Health. You can choose to connect Android Health Connect or Apple Health on iPhone so the coach can see your sleep, workouts and resting heart rate. It is optional, Formm only reads that data, you can disconnect at any time, and it is used to coach you, not to diagnose anything. It is not medical advice.

Voice. You can choose to dictate meals or talk to the coach by voice. Your speech is processed by our voice provider to make that work. Voice is a convenience, and the coach can mishear or get things wrong, so check anything that matters.

6. How you may use Formm

You agree not to:

use Formm unlawfully, or to harm, harass or deceive anyone,
try to break, overload, reverse engineer or gain unauthorised access to the service or other people's data,
use the service to build a competing product or to train or test other automated systems,
put in content you have no right to use, or content that is abusive or unlawful,
try to get around limits or security features.

7. Our content and your content

Formm, its design, the coach's style and the app's content belong to us or our licensors. We give you a personal, non-exclusive, non-transferable licence to use the app for your own training.

You keep ownership of the information you put in. You give us permission to store and process it to provide Formm, as described in the Privacy Policy.

Exercise pictures and step-by-step instructions are supplied by RepDB: Exercise data by RepDB (repdb.co). Some other exercise demonstration images are supplied by a third party (ExerciseDB by AscendAPI). The terms of those providers may apply to them.

8. Third-party services

Formm relies on third-party services (for example for hosting, sign-in, language models, voice and exercise media). We are not responsible for services we do not control, although we remain responsible to you for ours as the law requires.

9. Price and payments

Formm has a free plan and a paid plan, Formm Pro.

Free plan. You can use Formm without paying. It includes logging your workouts, your history and personal bests, swapping a move in your plan, and one new session built by the coach each week.

Formm Pro. Adds a new session built for you every day, chat with the coach, daily and session notes, food logging by describing or speaking a meal, form tips and live voice conversations (up to 40 minutes a month). Fair-use limits apply to each feature so that the service stays reliable for everyone.

Price. Formm Pro costs £9.99 a month or £59.99 a year, including VAT. The price shown in your app store (Google Play or the App Store) when you subscribe is the price you pay. If we change a price, the new price applies from your next renewal after we have told you, and you can cancel before it does.

Free trial. New subscribers get a 7-day free trial. When the trial ends it turns into a paid subscription automatically, at the price you chose, unless you cancel before it ends. We show you the date the trial ends before you start it, and we email you shortly before it ends. Your app store checks your payment method when you start the trial.

Payment and renewal. Payment is taken by Google Play (on your Google account) or the App Store (on your Apple Account), depending on which app you downloaded. Your subscription renews automatically each month or year until you cancel.

Cancelling. You can cancel at any time in your app store (on Android, open Google Play, tap your profile picture, then Payments and subscriptions, then Subscriptions; on iPhone, open Settings, tap your name, then Subscriptions), or from Profile in the app, which takes you there. Cancelling stops the next charge, and you keep Formm Pro until the end of the period you have paid for. Deleting your Formm account does not cancel your subscription, so cancel in your app store first.

Your right to cancel. If you live in the UK you normally have 14 days to cancel a digital service. Before you subscribe we ask you to confirm that you want your access to start straight away and that you understand you lose that right once you have started using it.

Refunds. Google Play and the App Store handle payments and refunds under their own refund policies. Nothing here limits your legal rights.

When Formm Pro ends. You move back to the free plan. Your training history, food log and other information stay in your account unless you delete it.

10. Availability and changes

We work to keep Formm available but cannot promise it will always be uninterrupted or error free. We may change, suspend or stop features, and we will give reasonable notice of changes that significantly affect you where we can.

11. Our responsibility to you

Nothing in these terms excludes or limits our liability for death or personal injury caused by our negligence, for fraud or fraudulent misrepresentation, or for anything else that cannot be excluded or limited by law, including your rights under the Consumer Rights Act 2015.

Otherwise, our total liability to you for any claim arising from your use of Formm is limited to the greater of the amount you paid us in the 12 months before the claim or £100.

12. Ending your use

You may stop using Formm and delete your account at any time. We may suspend or end your access if you break these terms or if we have to by law, and will tell you why where we can. Sections that by their nature should continue (such as 3 and 11) continue after the end.

13. Changes to these terms

We may update these terms. We will tell you in the app before important changes take effect. If you do not agree to the new terms you can delete your account.

14. Law and disputes

These terms are governed by the law of England and Wales, and the courts of England and Wales have jurisdiction. If you live in Scotland or Northern Ireland, you may also bring a claim in your local courts, and you keep the mandatory protections of your local law.
